Improving Access to Psychological Therapies Data Set Intermediate Database
Improving Access to Psychological Therapies (IAPT) Data Set submissions must be made using the latest IAPT Intermediate Database (IDB).
The structure of the IDB must not be altered in any way, and will be validated upon receipt of the submission to the SDCS Cloud. All tables must be present, even if some are not appropriate to the submission (based on the activity that has taken place). All fields listed in each table must also be present and field types must be exactly as specified.
The data items required to populate the IDB should be extracted directly from the appropriate local information systems, such as a clinical system or Patient Administration System.

Information Sharing to Tackle Violence Data Set XML Schema
The Information Sharing to Tackle Violence Minimum Data Set collects anonymised information on patients presenting at Accident and Emergency Departments for treatment as a result of a violent assault.
This information is to be shared with Community Safety Partnerships (formerly known as Crime and Disorder Reduction Partnerships in England) to reduce community violence.

File structure for the outgoing provider pre- and post-deadline extracts for the Improving Access to Psychological Therapies (IAPT) Data Set.
Once a provider has submitted a successful file and it has been processed, a pre-deadline extract can be downloaded to show what data would be carried forward if this was the final submission.
Further processing is later required to produce the post-deadline extracts (this takes place after the submission deadline).
Following post-deadline processing (once the submission window has closed), the SDCS Cloud generates a post-deadline extract for providers, which contains the elements of their data taken into data storage plus additional derivations from the submitted data.
PBCL Read Code to SNOMED Code Translation Table
A set of map tables translating the Read Codes in the PBCL EDIFACT laboratory report message to SNOMED CT codes to support transmission, reception, storage and retrieval of primary care pathology results.

Following implementation of the ICD-10 5th Edition on 1 April 2016, a number of errors were discovered in the printed ICD-10 5th Edition volumes published by the World Health Organisation. Some of these errors are replicated in the ICD-10 5th Edition eVersion.
We've produced a set of shared errata notes which can be imported into the eVersion.
There are now codes within ICD-10 5th Edition categories U06 and U07 that have been designated for emergency use. The descriptions of codes within categories U06 and U07 in the ICD-10 eVersion remain as ‘Emergency use of’.
We've produced a set of shared notes for the codes for emergency use which can be imported into the eVersion.
The shared errata notes and the shared notes for the codes for emergency use are available on Delen, along with instructions on how to import them into the eViewer.
OPCS-4 data files
These files are for NHS information departments and NHS system suppliers to upload to patient administration systems (PAS). They also help data analysts compare data collected using the current and previous versions of OPCS-4.
These files are not designed for training use.

SNOMED CT International General Dentistry Diagnostic refset
The General Dentistry set of diagnostic terms is designed to provide dentistry with a relatively easy to implement set of terms that covers the vast majority of care provided in the dental field. It does not attempt to meet the needs of dental sub-specialties, but may prove to do so for a significant amount of the care provided by these specialists.
Releases of this refset include content from the SNOMED CT International Edition.

Legally restricted patient confidential reference data
The technical preview code list was originally drafted to support a refresh of the ISB 1572 Sensitive Data standards and for no other purpose.
The legal basis for patient identifiable data and sensitive information changed with the introduction of the Health and Social Care Act 2012. However, the legal and policy context and interpretation for sensitive and/or legally restricted data has not been fully established due to the changes in legislation since the original standard was released.
As a result, this supporting code list has remained static since April 2016. NHS Digital will update the codes once an updated policy and instruction is provided by our commissioners.
NHS National Laboratory Medicine Catalogue
The National Laboratory Medicine Catalogue is a catalogue of SNOMED CT coded laboratory medicine test requests intended for use by Pathology services and their users across the NHS, and is still under development under the ownership of the Royal College of Pathologists.
The status of the National Laboratory Medicine Catalogue is ‘Technology Preview’, as defined in the IReS Terminology Product Development Lifecycle (external PDF). This means that trial implementation is encouraged to evaluate utility and safety, and to identify possible design improvements. Full live deployment is not recommended and will be at the user's own risk, where such risk is permitted by other governance processes.
The catalogue has not been updated since April 2014. Implementations may have to change if the product design changes in light of experience. If the product is found to be only minimally used or useful, or unsafe, then NHS Digital as the UK National Release Centre for SNOMED CT is under no obligation to continue releasing this product.

Further processing is later required to produce the mid-window and final post-deadline extracts. This takes place after the provisional procesing deadline for the mid-window extract, and after the final submission window closes for the post-deadline extract.
Following mid-window and post-deadline processing (during the submission window and once the submission window has closed), the SDCS Cloud generates mid-window and post-deadline extracts for providers. These extracts contain the elements of the provider data taken into data storage and additional derivations from the submitted data.
The Read Codes Drug and Appliance Dictionary
The final release of this item was on 1 April 2016.

SNOMED CT UK Query Table and History Substitution Table
Releases of this item contain 2 related derivatives of the SNOMED CT UK Edition to aid the processing of inactive content within systems and data repositories.
The History Substitution Table provides a single table identifying valid substitutions for concepts that are now inactive, along with metadata about that substitution; it is computed using the historical relationships within the main SNOMED CT release but presented as a simpler look-up table.
The Query Table is an extension to a normal SNOMED CT transitive closure table, taking into account the History Substitutions Table and other data. It provides a mechanism to achieve optimal data retrieval, regardless of inactive and active concept status in data or query definitions. The Query Table is recommended as a mechanism for ensuring inactive content is still returned in searches, and is specifically recommended by the Joint GP IT Committee for use in primary care systems.

SNOMED CT UK Data Migration Workbench
Releases of this item contain the SNOMED CT UK Data Migration Workbench application, which includes its database files pre-loaded with the latest versions of SNOMED CT, CTV3, READ, OPCS-4, ICD-10 and the crossmaps between them.
Releases also include an installation guide, a user guide and a "Read Me.txt" file. Read the "Read Me.txt" file before installing the application.

NHS UK Read Pathology Bounded Code List
The Pathology Bounded Code List (PBCL) was developed to provide a defined or bounded subset of Read codes for use in laboratory to GP messaging. Use of the PBCL ensures that there is common understanding by both sending and receiving clinicians of the laboratory investigations and results.
The PBCL continues to be supported following the final release of Read CTV3 in April 2018 but will only be updated on an ad hoc basis where there is an identified national need. Subscribers will be notified in advance of any changes.

SNOMED CT International Odontogram refset
The SNOMED CT Odontogram refset was developed from 2015-2017 as a tool to exchange basic oral health status information between dental clinicians using a standardized format and to support gathering of research and quality data based on the most common visualizations for clinicians worldwide.
Releases of this refset include content from the SNOMED CT International Edition.

Maternity Services Data Set Intermediate Database

Organisation Reference Data (postcode data only)
In line with the modernisation of Organisation Reference Data distribution channels and formats, the content in releases of this item has changed. Releases from October 2025 are quarterly, and only contain postcode data.
Reference data for organisations and their sites across health and care are available via the ODS Data Search and Export (DSE) tool. This tool lets you build and save advanced searches, and provides predefined reports that replicate the CSV files included in previous releases of this item, covering practitioner, organisation and site data.
DSE includes an API endpoint that supports automated downloads of predefined reports in bundles. You can use this tool as an alternative to a TRUD item subscription, except for postcode data – which ODS are looking to include in a future version of DSE. For details, see the help and guidance available on the DSE pages.

Post-deadline extracts are generated at the end of each month using the most recent valid file submitted before the submission window closed. For example, if October activity is submitted in November, it will only be available as a post-deadline extract after the November submission window closes.
For resubmissions, the updated post-deadline extract is not available until the window you resubmit in has closed. Until then, the extract will still show the original submission. For example, if October activity is resubmitted in December, the extract will show November’s submission until the December window closes. After that, it will update with the new data.

- many:one ‘backward’ crossmaps from Clinical Terms Version 3 to Read Version 2

Releases also include experimental SNOMED CT Navigational Subsets by which simulacra of the Read Version 2 and Clinical Terms Version 3 taxonomies may be recreated as views on the full SNOMED CT content.

Mental Health Services Data Set Commissioner extract XML Schemas
File structure for the outgoing Commissioner extract for the Mental Health Services Data Set (MHSDS).
Extracts are available for commissioners following post-deadline processing, and after they have been made available for providers. Commissioners will only see these once they have been loaded into the data warehouse. Their view will only show pseudonymised data.

SNOMED CT UK Monolith Edition, RF2: Snapshot
Each release of this item contains an integrated, fully merged set of the SNOMED CT UK Clinical Extension, the UK Drug Extension, the UK Pathology Extension and the International Edition, as a single set of files in Release Format 2 (RF2).
The merged tables are post-processed to resolve duplicates and conflicts that can occur when separate snapshots are combined.

NHS National Interim Clinical Imaging Procedures
The NHS National Interim Clinical Imaging Procedures are a set of procedure codes and descriptions for use in the representation of clinical imaging procedures, intended for use in all Imaging Department information systems.

dm+d XML transformation tool
Use this tool to read the XML files in releases of item NHSBSA dm+d in category NHSBSA dictionary of medicines and devices and convert the content to both Microsoft Excel and comma-separated value (CSV) files.
The tool:
- runs on Microsoft Windows (fully tested on Windows 7)
- runs from the C: drive - you may need local administrator permissions
- requires Microsoft Excel (tested with Excel 2010)
- uses a Microsoft Excel macro-enabled spreadsheet
No NHSBSA dm+d releases are included with the tool and must be downloaded separately.
SNOMED CT UK Clinical Edition Cross-map Historical Files, RF2: Full
Retired cross-map RF2 files, Full release type only.
There is a release per retired cross-map containing the extended map reference set release file and associated documentation.
The data contained within the releases are for reporting or data warehousing - the maps should not be used for live data processing. Active cross-maps can be found in the main Clinical Edition RF2 releases.

NHS Read Browser
NHS UK Read Codes release data (all versions) in browsable form. This includes English and Scottish variations of the Tri set browser. One directional cross maps from all versions of Read to ICD-10 and OPCS-4 can be viewed on an individual basis in the browser.
The last scheduled release of Read Version 2 and Clinical Terms Version 3 was April 2018. In line with the retirement of Read Version 2 and Clinical Terms Version 3, we no longer support or maintain this product.

VMP core SNOMED codes uplifted to a UK extension code - test file
Several VMP core to UK extension code changes took place on 26 to 27 May 2021.
This set of dm+d files are from 3 May 2021 (before the changes) to allow you to use these files along with a new set of dm+d files (after the changes, so from June 2021 onwards) to test for the upcoming changes planned for 2023 for all VMPs to change from a core SNOMED code to a UK extension code.

SNOMED CT to OPCS-4.9 Mapping File Patch Release
An unscheduled interim 28.1.2 mapping patch release of the SNOMED CT to OPCS-4.9 map (single Snapshot file only) is now available for download.
This is intended as data substitution mapping file to enable NHS system suppliers to comply with Information Standards Notice (ISN) for DCB0084: OPCS Classification of Interventions and Procedures which is mandated for NHS implementation on 1 April 2020.

IAPT post-deadline extract XML Schema release 2.1.14
File structure of the provider post-deadline extract for IAPT version 2.1.
This extract XML Schema aligns with the IAPT Enhanced Technical Output Specification version 2.1.14 and includes two new derived data items (“SickpayIndicator_First” and “SickpayIndicator_Last”) to reflect the first and last Statutory Sick Pay Receipt Indicators (SSPINDs).
This latest version replaces version 2.1.12 and should be used for any extracts post version 2.1.12.

CSDS post-deadline extract XML Schema release 1.5.21
File structure of the provider post-deadline extract for CSDS.
Extracts in this format will be sent up to and including the June 2022 close of the submission window. After that date extracts will be sent in the updated 1.5.27 format that includes new data items to reflect Integrated Care Boards.
XML Schema element names used for derived data items in CSDS 1.5 have been realigned and renamed for consistency with similar derivations in other tables across the dataset.
Some of the derived data items in CSDS 1.0 have different XML Schema element names in CSDS 1.5.
Use the latest version of this Schema when working with CSDS 1.5 post-deadline extracts.

The only update in this release compared to the previous 1.6 release is a field name change from “LocalPatientId” to “LocalPatientID” in table “CYP008OverseasVisitorChargCat”.

This release is useable from May 2022.
Included are changes to reflect the capture of Gender, Medical History and amendments to align with the NHS Data Model and Dictionary. See the included production log for details.
This release supercedes the previous release of the IDB and resolves the issue of entered text being padded with spaces up to the maximum field length.
Use this release for all 2.1 submissions going forward.
IAPT 2.1 submissions start from May 2022 for April 2022 reporting period data.

This amended release incorporates format changes to 6 data items to resolve submission validation errors identified at go-live. All table and field names remain the same. Please see the production log included in the release for details of the changes.

An internal IDB technical issue that was padding entered text with spaces to the maximum field length has been resolved.
The format of field SMHEPCCharge in table MHS517SMHExceptionalPackOfCare has been changed from number (Decimal 9 Precision 2) to string (Length 10) for easier population.
Table names, field names, field types (with the exception of field SMHEPCCharge) and sizes are identical to the previous release.
All Provider/Supplier processes and systems that have been developed to populate the IDB should not be affected and will work with this release.

MHSDS post-deadline extract XML Schema version 6.0
Post-deadline extracts in this format will be available up to 14 October 2024. After that date extracts will use the updated 6.0.3 Schema.
This extract XML Schema aligns with the MHSDS Enhanced Technical Output Specification version 6.0.2
The included production log contains separate tabs to show changes to previous preview release (Release 6.0.0.4 Preview 1) and changes to Release 5.0.42.
Release 7.0.1
This release is a minor technical update to the previous version 4.8 release of 2016.
The Product Status remains TECHNOLOGY PREVIEW.
Changes are restricted to the SNOMED code lists only, to bring these lists back in line with their primary design requirement that they should not contain inactive SNOMED codes.
All SNOMED codes listed in the previous version 4.8 of this product but that have since become inactive are therefore removed and replaced with semantically equivalent but still active codes (where possible).
You should continue to identify all inactive SNOMED codes subject to the same legal restrictions by inference from the supplied list of active-only codes, using SNOMED’s history mechanisms as per their usual practice when processing any other similar code list specification.
Active codes relating to either Assisted Conception or to Sexually Transmitted Diseases will not be included if they were only released as new SNOMED codes after 2016.
For example:
838317007 Bursitis caused by late syphilis
was only released as a SNOMED code in 2020, and so is still not listed within the LRPCD_STD_v4.8.1_ClusterMembers_SCT_20220801000000 product.
Further LRPCD product major releases will include this kind of more comprehensive content review and update.

Post-deadline extracts in this format will be available up to 14 February 2025. After that date extracts will use the new 6.0.4 XML Schema.
This post-deadline extract XML Schema aligns with the MHSDS Enhanced Technical Output Specification version 6.0.3.

File structure of the provider mid-window and post-deadline extract for MSDS version 2.0.
This Schema will be used for both the mid-window and final post-deadline extracts. Note - The mid-window extract will be available from June 2023.
This release provides compatibility between version 2.0.28 of the post-deadline extract XML Schema and all later versions of the MSDS Technical Output Specification.
The maximum length of UniqSubmissionID elements is increased to 10 digits.
The Schema does not restrict element lengths so check the production log for increased storage requirements.

There is an issue with this release.
This release stores entered text with padded spaces up to the maximum field length. This does not affect the structure or population of the IDB, but could impact downstream processing after submission to NHS Digital.
A new release is now available that contains identical table and field names, field types and sizes – the only difference is that entered text is not stored with padded spaces up to the maximum field length.
This is an internal change to the IDB concerning the physical storage of data after entry. All Provider/Supplier processes and systems that have been developed to populate this release should not be affected and will work with the new release.

The ICD-10 5th Edition Codes and Titles and Metadata File GENDER_MASK metadata field has been changed from an absolute check to a scrutiny check in the updated ICD-10 5th Edition Codes and Titles and Metadata File Specification document. This change is explained in the Addendum (Amd 10/2014) on the NHS England website.

This release is useable from 1 July 2023.
This is an update of the previous 20 January 2023 release (now withdrawn). It corrects the cardinality of the data group EXPECTED DATE AND TIME OF TREATMENT to match the NHS Data Model and Dictionary.
The Information Standard was approved by the Data Coordination Board as DCB0092-2062 : Commissioning Data Sets: Emergency Care Data Set.
Previous versions supplement the existing CDS062 version and allows submission of CDS011 (Emergency Care) CDS Type only.

SNOMED International rationalisation of Unit of Measure concepts
SNOMED International is making changes to 2 overlapping hierarchies:
- 732935002 |Unit of presentation|
- 408102007 |Unit dose (qualifier value)|
These changes will result in the inactivation of duplicate concepts across both sub-hierarchies. Both sub-hierarchies are subtypes of 767524001 |Unit of measure (qualifier value)|.

Some affected "Unit of product usage" concepts are used in the NHS dictionary of medicines and devices (dm+d) as Units of Measure.

Aligning the SNOMED CT UK Drug Extension model with SNOMED International
NHS England posted an update on Delen in December 2025 about the planned phase 3 and phase 4 changes to the SNOMED CT UK Drug Extension. NHS England asked for feedback on its plan to publish the changes as a single, combined release. It did not receive any concerns by 15 January 2026, so it will proceed with this approach.
To help NHS England understand the impact of these changes, it plans to publish a survey in March 2026 with test files. This will let you review the phase 3 and phase 4 changes and provide feedback.

Aligning the SNOMED CT UK Drug Extension model with SNOMED International
Further to the news posted on Delen regarding the planned phase 2 changes to the SNOMED CT UK Drug Extension, NHS England will go live with the phase 2 changes in release 41.5.0, due 18 February 2026.
NHS England will release a user survey and accompanying test files for the phase 3 (AMP) and phase 4 (AMPP) changes to understand user impacts. They plan to go live with the phase 3 and phase 4 changes together in a joint release.

Duplicate map entries in SNOMED CT to ICD-10 mapping file 41.2.0
NHS England has identified an issue in the SNOMED CT to ICD-10 mapping file included in the SNOMED CT UK Edition 41.2.0 release. The file der2_iisssciRefset_ExtendedMapUKCLFull_GB1000000.txt contains duplicate map entries.
A corrected SNOMED CT to ICD-10 mapping file will be released on 24 December 2025 as part of the 41.3.0 release.

Increasing SNOMED International description length limits
SNOMED International has confirmed the planned change to increase the maximum length of Fully Specified Name (FSN) and Synonym descriptions from 255 characters to 4,096 characters.
This change will be implemented in the 1 July 2026 International Edition release and will come into effect in the October 2026 UK Edition release.

NHS England is seeking views from SNOMED CT users in the UK through a survey to help identify if additional information or support may be needed to prepare for the change, and to understand the most effective ways to communicate and share that information.

Aligning the SNOMED CT UK Drug Extension model with SNOMED International
Following the phase 1 changes implemented in October 2025, NHS England is now progressing with the phase 2 changes.
The NHS England web page on aligning dm+d VMPP class medicinal products to the SNOMED International drug model has been updated with information about the changes, including examples and diagrams to help explain them.
Test files with the phase 2 changes to the SNOMED CT UK Drug Extension are available on Delen.
The changes are planned to be implemented in the first quarter of 2026.
In preparation, NHS England is asking SNOMED CT UK Drug Extension users to complete a survey so they can understand the effect the changes may have. Your feedback will help NHS England work with you to implement these changes.

The plan is to:
- release a test file with the Phase 1 changes for SNOMED CT UK Drug Extension users in July 2025
- go live with the Phase 1 changes in the SNOMED CT UK Drug Extension in September 2025

Draft changes from OPCS-4.10 to OPCS-4.11 open for feedback
The next update to the OPCS-4 Classification of Interventions and Procedures – version 4.11 – will be implemented across the NHS on 1 April 2026.
OPCS-4 is an information standard published under the Health and Social Care Act 2012. It is updated every 3 years to reflect current NHS clinical practice. The classification supports aggregate data for statistics and reimbursement.
The Terminology and Classifications Service at NHS England has produced a draft summary of the core changes between OPCS-4.10 and OPCS-4.11. Users of the classification are invited to review the changes and submit feedback.
The draft summary of core changes and the feedback form are available on the OPCS-4.10 to OPCS-4.11 feedback page on Delen, the Terminology and Classifications Service's collaboration workspace.
If the changes are approved by the Data Alliance Partnership Board (find out more about the governance of information standards), the OPCS-4.11 Information Standard Notice will be published in November 2025. This is to prepare for implementation in the NHS from 1 April 2026.

Have your say on plans to remove Concept Non Current indicators from SNOMED CT
SNOMED International plans to stop using Concept Non Current (CNC) indicators in the International Edition of SNOMED CT from 1 January 2026.
At the moment, CNC indicators are added to active descriptions when a concept becomes inactive. Under the new plan, this will no longer happen. All current CNC indicators will also be made inactive, but they will remain in place as inactive components.

SNOMED International proposal to increase description length limits
This is an update to the post on Delen in January 2025 about a proposal from SNOMED International. The proposal is to increase the maximum length of Fully Specified Name (FSN) and Synonym descriptions from 255 to 4,096 characters.
SNOMED International has published more details about the proposal. You can read these in their PDF document: March 2025 date approval MF BN 20250326 v1.00.pdf.

This change will affect UK users who use content from the Medicinal Product Hierarchy in either the SNOMED CT International Edition or the SNOMED CT UK Edition.
It will also affect FSNs created in the SNOMED CT UK Drug Extension.
The proposal will not affect Preferred Terms (PTs) in the SNOMED CT UK Drug Extension or the NHS Dictionary of Medicines and Devices (dm+d). We're not planning any changes to the dm+d as a result of this proposal.
SNOMED International has postponed the proposed implementation date to July 2026. This gives users more time to prepare. The change will take effect in the October 2026 UK release.

The change will affect UK users of content from SNOMED International’s Medicinal Product Hierarchy, whether taken from the SNOMED CT International Edition or the SNOMED CT UK Edition. However, it will not affect the NHS dictionary of medicines and devices (dm+d), and we do not plan to make any dm+d changes in response to this proposal.
We are reviewing how this change might affect FSNs in the SNOMED CT UK Drug Extension, but we do not expect it to affect Preferred Terms (PTs) in the UK Drug Extension.
SNOMED International currently plan to implement this change in January 2026, with it taking effect in April 2026 UK releases.

No ODS weekly releases over Christmas and New Year
There will be no releases of item NHS ODS weekly prescribing-related data for the 2 weeks over Christmas and New Year. Both production dates, 25 December 2024 and 1 January 2025, fall on bank holidays.
The last release of this item before Christmas is on Thursday 19 December 2024. Weekly releases will resume on Thursday 9 January 2025.
Share your views on the dm+d and SNOMED CT UK Drug Extension release cycles
As part of the UK Medicines Terminology Futures work, NHS England is seeking views from users of the dm+d and SNOMED CT UK Drug Extension, to understand preferences for release timings of these terminologies, and potential impacts if timings were changed.
We're seeking views about release timings to ensure the NHS is meeting user needs, that terminology information is cascaded at times that align with other dependencies, and to reduce burden where possible.
This is an opportunity to highlight your preferences and does not mean release timings will change. If any release timings are to change, further impact analysis will take place to assess implementation options. Any change would not take place until at least summer 2025.

Long descriptions in the April 2024 release
SNOMED International included 2 descriptions in the April 2024 release which were longer than 255 characters. The May 2024 release shortened these descriptions to within the current 255-character limit.
Subscribers to items in the SNOMED CT UK Edition category will only see the shortened descriptions in release content. UK users who use the latest 'Snapshot' file of the International Edition will also only see the shortened descriptions.

SNOMED CT International Edition: long descriptions
This announcement affects subscribers to the SNOMED CT International Edition item.
SNOMED International included 2 descriptions in the April 2024 release which were longer than 255 characters. The May 2024 release shortened these descriptions to within the current 255-character limit.
Subscribers to items in the SNOMED CT UK Edition category will only see the shortened descriptions in release content. UK users who use the latest 'Snapshot' file of the International Edition will also only see the shortened descriptions.
