NHS Data Migration
Releases of this item contain products intended to support the GP Summary Care Record, Systems of Choice and Data Migration programmes. Specifically:
- many:one ‘forward’ crossmaps from the England and Scotland releases of Read Version 2 to Clinical Terms Version 3, from Read Version 2 to SNOMED CT, and from Clinical Terms Version 3 to SNOMED CT
- many:one ‘backward’ crossmaps from Clinical Terms Version 3 to Read Version 2
- tables assigning NHS Care Record Elements to all Read Version 2, Clinical Terms Version 3 and SNOMED CT codes - these mapping tables are distributed in a non-standard file format, together with supporting documentation
Releases also include experimental SNOMED CT Navigational Subsets by which simulacra of the Read Version 2 and Clinical Terms Version 3 taxonomies may be recreated as views on the full SNOMED CT content.

Following consultation, the final release of the Read Version 2 to SNOMED CT mapping tables was April 2020. Releases of this item are now unmaintained static data.

NHS Read Browser
NHS UK Read Codes release data (all versions) in browsable form. This includes English and Scottish variations of the Tri set browser. One directional cross maps from all versions of Read to ICD-10 and OPCS-4 can be viewed on an individual basis in the browser.
The last scheduled release of Read Version 2 and Clinical Terms Version 3 was April 2018. In line with the retirement of Read Version 2 and Clinical Terms Version 3, we no longer support or maintain this product.

NHS UK Read Pathology Bounded Code List
The Pathology Bounded Code List (PBCL) was developed to provide a defined or bounded subset of Read codes for use in laboratory to GP messaging. Use of the PBCL ensures that there is common understanding by both sending and receiving clinicians of the laboratory investigations and results.
The PBCL continues to be supported following the final release of Read CTV3 in April 2018 but will only be updated on an ad hoc basis where there is an identified national need. Subscribers will be notified in advance of any changes.

VMP core SNOMED codes uplifted to a UK extension code - test file
Several VMP core to UK extension code changes took place on 26 to 27 May 2021.
This set of dm+d files are from 3 May 2021 (before the changes) to allow you to use these files along with a new set of dm+d files (after the changes, so from June 2021 onwards) to test for the upcoming changes planned for 2023 for all VMPs to change from a core SNOMED code to a UK extension code.

This release includes clinical code lists for In Vitro Fertilisation and Sexually Transmitted Diseases across the following terminologies and classifications:

- ICD-10
- OPCS-4
ISB 1572 is subject to review by the Standardisation Committee for Care Information.

Aligning the SNOMED CT UK Drug Extension model with SNOMED International
NHS England posted an update on Delen in December 2025 about the planned phase 3 and phase 4 changes to the SNOMED CT UK Drug Extension. NHS England asked for feedback on its plan to publish the changes as a single, combined release. It did not receive any concerns by 15 January 2026, so it will proceed with this approach.
To help NHS England understand the impact of these changes, it plans to publish a survey in March 2026 with test files. This will let you review the phase 3 and phase 4 changes and provide feedback.

Aligning the SNOMED CT UK Drug Extension model with SNOMED International
Further to the news posted on Delen regarding the planned phase 2 changes to the SNOMED CT UK Drug Extension, NHS England will go live with the phase 2 changes in release 41.5.0, due 18 February 2026.
NHS England will release a user survey and accompanying test files for the phase 3 (AMP) and phase 4 (AMPP) changes to understand user impacts. They plan to go live with the phase 3 and phase 4 changes together in a joint release.

Draft changes from OPCS-4.10 to OPCS-4.11 open for feedback
The next update to the OPCS-4 Classification of Interventions and Procedures – version 4.11 – will be implemented across the NHS on 1 April 2026.
OPCS-4 is an information standard published under the Health and Social Care Act 2012. It is updated every 3 years to reflect current NHS clinical practice. The classification supports aggregate data for statistics and reimbursement.
The Terminology and Classifications Service at NHS England has produced a draft summary of the core changes between OPCS-4.10 and OPCS-4.11. Users of the classification are invited to review the changes and submit feedback.
The draft summary of core changes and the feedback form are available on the OPCS-4.10 to OPCS-4.11 feedback page on Delen, the Terminology and Classifications Service's collaboration workspace.
If the changes are approved by the Data Alliance Partnership Board (find out more about the governance of information standards), the OPCS-4.11 Information Standard Notice will be published in November 2025. This is to prepare for implementation in the NHS from 1 April 2026.
