NHS Digital is responsible for the publication and maintenance of the NHS Information Standards ICD and OPCS-4 on behalf of the United Kingdom, as well as other products to support the implementation of these clinical classifications and derivative products.
The NHS Digital Terminology and Classifications Delivery Service sets the national clinical coding standards for these classifications in England and is the definitive source of these standards and guidance.
Releases of items within these categories contain essential data files and associated products to support the clinical classifications, along with guidance on how to use them effectively.

Draft changes from OPCS-4.10 to OPCS-4.11 open for feedback
The next update to the OPCS-4 Classification of Interventions and Procedures – version 4.11 – will be implemented across the NHS on 1 April 2026.
OPCS-4 is an information standard published under the Health and Social Care Act 2012. It is updated every 3 years to reflect current NHS clinical practice. The classification supports aggregate data for statistics and reimbursement.
The Terminology and Classifications Service at NHS England has produced a draft summary of the core changes between OPCS-4.10 and OPCS-4.11. Users of the classification are invited to review the changes and submit feedback.
The draft summary of core changes and the feedback form are available on the OPCS-4.10 to OPCS-4.11 feedback page on Delen, the Terminology and Classifications Service's collaboration workspace.
If the changes are approved by the Data Alliance Partnership Board (find out more about the governance of information standards), the OPCS-4.11 Information Standard Notice will be published in November 2025. This is to prepare for implementation in the NHS from 1 April 2026.
